I sometimes step out very briefly but I also make sure that I give exams that are such that students will have little opportunity to gain very much by some hasty exchange of information during a moment of my inattention.

Some colleges have standard "honor systems" policies which students understand... ie virtually all tests at these schools are not monitored...

The only time is if he has TAs in the room and even then most Universities frown on it.
