It originally came from the Sign Language alphabet - I is the pinky up, L is the pointer and thumb, and all three together looks like the letter U - and it originally meant "I Love You".
Since then, the surfer culture in Hawaii has modified it (pinky and thumb out, pointer in) to mean "hang loose" or "keep it cool." Another modified version (pinky and pointer up, thumb in holding the other fingers) symbolizes devil's horns and is used in heavy metal culture.

Hang loose is just the thumb and the pinky. When people do the hand sign that you mentioned, it's like a heavy metal "ROCK ON" thing... or devil's sign.

usually its I love you but it could mean various things
the pinky is the I
the L shaped made with your pointer and thumb is the L for Love
and the shape made with the pinky, knuckels,and pointer is the U
