i tryed doin all the quest but i cant find it

You have to scream, "I'M STUPID!!!"

Then it appears in your inventory.

you need to do the fight pits inside the volcano/cave on karamja and you need to defeat alot of high level monsters. i think around lvl 200

but its very hard to get and you cant get it from quests
