Diaper Cakes are always a good gift, especially since most people tend to overlook the fact that mothers desparately need diapers. You could definetly make it special with a few small toys, bibs, bottles, burp cloths, etc.
Another one that I made for a friend who had her baby in December was a fleece carseat cover. Just measure the length of her chosen pumpkin seat (over the bar) (you could even do this at the store). buy some fun fleecy fabric in 2 complimentary colors (make sure that you cut it 5-6 inches too big). Cut 4-5 inch slits all the way around and tie the two pieces together. That is really cheap. You could also make a blanket this way. Two matching ones is a good idea.
You could make your own recieving blankets and matching burp cloths. Take one printed SOFT piece of cloth, and one complimentary solid color (also soft) cut them to the appropriate size and turn them print side towards the inside. Sew all the way around except for a small 2-3 inch piece. Turn it right side out and poke a toothbrush end into the corners. Fold the 2-3 inch piece in and stich that by hand.. Press with a warm iron. You could make dozens of these for minimal cost, and again you could make recieving blankets and burp cloths to match.
A small quilt is also not too difficult. But more so than the others. I would decide on 16 same size squares of pastel colors, sew them together right side in (like the burp cloths) in strips 4, 4 and 4. then sew the strips together. Cut longer strips for the sides and top and bottom, sew them on as well. ( I have simple patterns and ideas, if you would like to email me, it is a little more complicated though, I dont reccommend this unless you are pretty handy with a sewing machine) Stick the cotton batting inside and pick a color for the back. Sew together. More complicated and time consuming, but you can find simple quilt patterns anywhere.

What I really appreciated were things that I used on a daily or very frequent basis....it saved me on many trips to the store when I was so sleep deprived from feedings every 2 - 3 hours and being up all night.
I really like the bathtub idea and filling it up or you could get a small toddler-sized wicker laundry basket and fill it up with the following
1. Newborn size diapers or size 1 diapers (diaper rash cream also...I suggest A&D ointment)
2. Newborn gentle or sensitive skin baby wipes
3. Mylecon Gas Drops (HUGE lifesaver to have on hand especially at 2:00a.m. and the baby has gas and there's no store that is open!)
4. Infant Tylenol drops
5. Nasal aspirator, even though the new Mom will be sent home with one an extra is nice to have in the diaper bag
6. Little trial size bottle of 50+ SPF sunblock as yes, babies can get burned even in the car with the sun shining in and summer is just around the corner
7. Car Sunshade for the passenger rear window
8. Infant nail clippers
9. Teething Rings
10. Baby shampoo
11. Baby Bath (I recommend Huggies Extra Sensitive for newborns)
12. Baby Lotion
13. Infant washcloths
14. Hooded Towels (I personally never had enough!)
15. Newborn or 0 - 3 size white socks (those puppies get lost so easily in the wash due to their size....you can never have enough)
16. Burp Cloths
17. Lap Pads (these are a lifesaver for when you hold a newborn so you don't get your lap soaked due to a leaky diaper)
18. Bibs
19. Q-tips
20. Receiving blankets

You can do a diaper cake or what I have done in the past is just get some of the following items in an inexpensive basket from a craft store with filler and a nice bow, they is also the wrap. I stuff it full of diapers, lotion, butt paste, baby wash, tub toy, baby powder, pacifier, baby oil etc, you know the practical things mothers can use everyday.
You get the freedom to mix and match different items that would be more suitable to your cousins taste.
Another "holder of sorts can be a baby bath tub. Depending on your budget you can find one for about $20.00
With the exception of the diapers, all the other things you can buy 2 of and it will work out to be about $20.00. Hit Babies R Us, they surprisingly have bath stuff cheap or Walmart
Oh yeah, a good thing too is a pack of cloth diapers. They are inexpensive and they make great burp cloths. They are cheaper and larger than the conventional fancy ones

If you had time to learn how to knit (or already knew how) you could easily whip out a blanket. Just use thick yarn and large gauge needles, like 12s or 15s. To make it even quicker, pick out 3 or 4 different yarns and knit the strands together, as one. I've also made a blanket by purchasing one from a store, then sewing a silk edge all around it that I embroidered with different words (joy, love, whatever). I like the bib idea above too! not keen on cutesy gifts like diaper cakes and such, but I know a lot of people who love things like that!

I had a friend who got cloth diapers as burp clothes and they just sewed really cute ribbon across each end that had sports items on it. If it is a girl there are all kinds of pretty ribbons out there. Ribbon is cheap, less than $1.00 a yard and pre-folded cloth diapers 12 in a pkg are like $12.00 at Wal-mart. I thought it was a pretty cool idea. If you had extra funds I would say have the baby's name put on it. That can get more expensive.

Being a new mom again I would suggest Blankets or diapers I didnt get allot of diapers and they arekind of pricy when you have ot start out buying them I love the pampers swaddlers they are more expensive but they are worth it ot me i have tried the others and they just dont cut it but that is each its own opnion try that out. I think the diaper cakes get a little expensive to make. Blankets are the greatest specially if they have a theme that your arent able to find allot in I love western and they are hard to find so I made all of mine for the most part.

Try one of the new tie blankets. No sewing is needed, they are cute and useful and will probably last many years. Make it is a smaller size more suitable for baby.
