The truck has 195.000 miles.
The trans is locked in a high gear. The truck can be driven by riding the clutch.
The shifter stick fells totally disconnected but still in place and can moved around freely. I cannot shift out of that high gear 3rd, 4th or maybe even 5th gear. I'm 60 years old, mechanically inclined and am planning to repair this myself. What is the most common reason for this happening? Could it be something as simple as a worn-out washer or spacer? Is there a chance I may be able to repair it without removing the trans from the vehicle?

roll pin has sheared off where shift rail goes into shifter fork . The trans will have to come out and be pulled apart to fix, but the roll pin and sealer won't cost much. labor will run up the bill though.
check bearings for lay shaft and 5th gear while apart i have had several cases where these were in a failing state. the syncro's are usually still in decent shape at this mileage unless a complete heathen has been driving.for a long time.

Honestly I would start with the shifter tower. but, the trannies are notorious for taking a dump. If this is the original then you have gotten a good long life from it. Just pull it and get a remans or rebuild it yourself. it is a lot less expensive that way.

you are going to have to drop the trany to fix the problem sounds like bad bearing.

the first thing I would check is if the linkage was in tact
