it depends on where you from. In Oklahoma, where I'm from, if you're married, your husband's name goes on the birth certificate, regardless, unless and until there is a DNA test to dispute it or another man signs a parental release form.

To name someone on the birth certificate, when you're not married, that person would need to sign a parental release form, stating that he is the baby's father. I was really young when I had my first, and the father spot on his birth cerificate is blank. When we get back home, my husband is going to sign a parental release form, stating that he's my son's father, and have his name put on his birth certificate.

You don't have to lie, to leave it blank. I didn't lie to keep mine blank. I simply asked if I had to fill that part out. I was told no, it was my right to leave it blank. So I did.

i'm not really sure, but if you are if you know for a fact this person is the father i would try very hard to give my child his father last name. his mother should be willing to sign the certificate, if not you have had your last name all of your life be proud of that name and your child can wear that name with great pride. lets just hope he will step up to the plate and be a man about. good luck, you will be okay no matter what.

i don't beleive you're able to do a NYS call substitute withoug performing until now a choose. i might try calling the medical institution the place you have been born and seeing what they sugggest; and the NYS branch of necessary documents.
