Paula Patient doesn’t want her violent boyfriend to know she’s getting an abortion, so she asks Dr. Bob to send all communications to her post office box and to phone her only at work. What should Dr. Bob do, and what is the legal basis for your advice? If she tells Dr. Bob that not doing this will endanger her, would your answer be different?

Under current HIPPA laws doctor Bob cannot reveal any information, leave any messages on answering machines, or give any written information to anyone but the patient.
BUT give that situation it is up to the patient to supply the front office with all the updated information for their chart. In an office setting you cannot expect the office staff to know each persons personal life - and stop with each mailing and say that "this information goes to Sally's house and this information goes to her PO box".
So there is both patient and physician responsibility in HIPPA. If you have two numbers on the chart - they will call them, or two addresss -- you are not protecting yourself, and it is not the physician's responsibility to weed out your personal issues.

HIPPA would not allow DR.Bob to disclose any health info about the patient anyway.
