If these people have always been nice to you, did you favors etc and now expect a favor back but doing them the favor will permanently change your life?

Well, depends what kind of favor, if its changing you for the better then its ok but if not, then don't. Its not selfishness if ever you refuse.

If people do you favors, it is not always necessary that you reciprocate, nor its it wrong if you choose not to reciprocate.

i really never turn people down even at my own expense. I end up feeling guilty, it is just my nature.. have I been let down, you bet!!
