This noirish fantasia by David Lynch is ostensibly about an aspiring Hollywood actress (Naomi Watts) who is driven to hire an assassin to kill her lesbian lover then, overcome by guilt, kills herself. The first two thirds are all a dream, in which Watts plays an alternate self with a different name. The final third is reality, but then the non-dream story resumes only partly in the present tense. It also flashes back to events that took place before she went to sleep, and after she shoots herself.
Things are further complicated by having the surreal events if the dream story continue over into reality. A monster from the dream, for example, is explained later as being just a tramp, but then he releases cackling versions of the heroine’s grandparents who drive her to suicide.
The key pieces of symbolism are a blue box and a key, which the assassin says he will leave for her once he completes his job. Some think the box is a gateway to a parallel reality, making the actress a schizophrenic. But it can be successfully read as a Pandora’s box of buried guilt.
Ultimately, the dream is the heroine’s subconscious attempt to rationalise her failure in Hollywood. Just as Dorothy in The Wizard of Oz recycles the people she knows in her dream, so the heroine re-imagines all her rejections in terms of a vast conspiracy.

I got lots of spoilers for anyone who hasn't seen it here:
Anyway, the first part of the movie, when there still nothing too weird, when the two girls are living together and rita has amnesia, thats supposed to be a dream. when the cowboy comes in and says "hey pretty girl time to wake up", thats when the main character (naomi watts) wakes up from her dream and comes back to real life. the dream was supposed to be about how she wanted all things to be. she had that girl rita all for herself, was begginning a career as a successful actress and the movie director was having a terrible life. i guess the name confusion happens, because basicly everyone confuses some names in dreams. then in real life, everything was frustration for her, since rita is marrying the director adam kesher and she's lonelier than ever. eventually some stuff seem to hard to explain and u just gotta interpret them yourself, like the weird guy behind the rest. and the little people. finally i think club silencio was just a weird place she was imagining in her dream.
