Does anyone know the poem that Cameron Diaz reads the old man in the begging of "In Her Shoes"...?

This is the poem she reads the blind man that she has trouble reading to him...its something like Our Hearts mabey? Im not sure...I would really like the poem because I liked it a lot...please help it would do great to me.
