Well about the whole "are you ready thing," just listen to what your gut tells you.. and if he ends up not being the one and you love him than you wont ever regret what ever you did with him..
And as for your second concern.. I was like that when i with my boyfriend. and if the way you look bothers you, than do it in the dark. and almost always how we really look isn't as bad as we all think.. and since he loves you than he loves you for you and wont care what you look like..

If you and he have your own place, then you just need to work out your conflicted feelings about it first (they are very strong and will prevent you from having a good experience).
If you or he are living with parents, then you can safely hold off and I don't think anyone will be too dissapointed, and with the way you feel about it, It will just heighten your anxiety to try it.

I say dont do it untill you know your going to be together forever. And if you do do it, use saftey condoms are a must. I say dont give what your not ready to. Love`s memory is everlasting but love sometimes isnt are you ready for it if its not?
