you need to gradually stretch out the hole in your ear to gauge it. usually you buy plastic earing-like things that make the holes bigger. it can be painful at first. it's like having braces. it depends how long it takes, to how fast you change the sizes of the plugs. no, it can't get infected, your ear piercing is already healed.
hope this helped! :]

Well you start with a pierced ear and then gradual stretch the hole into a bigger gauge. It shouldn't "hurt" if done correctly but it isn't unusual for your ear to become red and warm. As long as you don't put objects in your ear that don't belong (and you ear piercing is fully healed before gauging) It shouldn't get infected

First thing, "gauge" is a unit of measurement, not an object. (I don't mean to be condescending, just letting you know).

In order to stretch your lobe, you'll need a taper (preferably stainless steel) that will gradual increase the hole's size to the next gauge. It's best to consult a piercer or friend who knows how to do this.

It is somewhat uncomfortable during the process, and depending on the degree of stretch, there will be some soreness for a day or two.

Finally, they can most certainly get infected if not cared for properly. Don't let anyone tell you otherwise.
