For now heres a quick summary I've done:
It begins with their graduation before skipping to the day before Kyo and Tohru leave home. The two of them are cleaning one last time and have already packed. A sweet moment is interupted by Tohru's stomach and it switches to Yuki and Machi. He gives her a key but when she says she doesn't want it he threatens to throw it out the window. She accepts it tells him she'll always catch up to him without a doubt and he kisses her. At the Sohma estate Ritsu is giving Kagura all his female clothes. He's cute his hair short and we learn he's finally asked her out on a proper date. Kagura is happy teaching and doesn't want to be tied down with anyone. Next is Kisa and Hiro. Kisa is upset that Tohru is leaving but Hiro tells her that if she has to cry tomorrow do it because of how much she favoured her. Then we go to Momiji, Haru and Rin. Momiji has decided that he's going to find a girlfriend better than Tohru so when she returns he can show her off. A running theme during these scenes is a small flash back of the first time they each met her before cutting to the next group. Saki and Kazuma are up next. Kunimitsu isn't thrilled at the idea of having Saki as their cleaner but he soon stops complaining. Rather than the first time Kazuma met Tohru we see a flash back of her carrying Kyo back home from vol 6. Uo is packing at home ready to leave. She talks on the phone to Kureno who is still walking with a cane. Finally we see the wonderful Aya who is still with Mii. (Are we noticing a pattern?) They're still running their shop together and we once more get a flash back before it cuts to Hatori and Mayu. Most vital thing to notice here is that Mayu's hair is now short suggesting she is finally dating Hatori. Mayu is still her old self though and they leave the cafe together. She mentions that it would be nice for the weather to clear up soon and we see just Hatori's flashack of Tohru. Akito is now openly wearing female clothes and is walking along the street with Shigure. He's given up being a writer and no longer has to hide his relationship with the head of the family. He's much more like the Shigure we first met, no longer dark and scheeming and he tells her she's a cunning child. We see the first time Akito built up the courage to go and visit Tohru in hospital before we return to Kyo and Yuki. They're finally able to talk to each other decently and Yuki give's Kyo some words of advice only to be told he didn't need to tell him such things. They part ways by calling each other a stupid cat and rat. Finally comes the moment that Yuki is able to tell Tohru his feelings. He explains how he thought of her as a mother. We see everyone for the last time as he speaks before he thanks her. As he says thank you for the last time he calls her Tohru. She smiled and their hands part. We see the words "I’m going but I’ll be back" and one last image of the four of them at the table before we skip to the future. Though its at a different angle than normally shown we see Shigure's old house. A small girl is singing to herself and carrying a toy cat and rat as she goes to the kitchen calling for her mother. She asks where "Toru-kun" and "Kyo-kun" are before getting told off. Her mother (who has hair suspiciously similar to Yuki's) tells her that she should call them grandma and grandpa but the girl says that they told her to call them that. Her father tells the little girl that the two are outside together. We see Kyo in clothes like Kazuma's and Tohru in western clothes walking together hand in hand. They're now adults and appear to live together with their son and his wife and child. The last page shows a cabinet of memories. Top shelf: Kyo's hat Yuki once gave her.
Bottom shelf: Kyo's beads in a dish, Tohru's zodiac models, a photo of Kyoko, Katsuya and Tohru, a photo of Kyo and Tohru wearing her muffler, a photo of their wedding day, a photo of their son in his old school uniform, two other photos and a group photo of Kyo, Tohru, Uo and Hana.

To the poster below ~ haha, sorry.... I thought that the series was over at ch. 134.... the endings keep seeming to end, and then another chapter comes out. I just never remember the actual chapter numbers, so I assumed that 136 was one of the last ones. Thanks for the info, I'm excited!
