Technically Sondheim is a lyricist/composer who uses books or storylines to create amazing musicals. He started out as a lyricist working as a protege under Leonard Bernstein and henceforth wrote the lyrices for Do I Hear A Waltz? (music written by Richard Rogers), The Mad Show (music by Mary Rodgers), West Side Story (music be Leonard Bernstein), Gypsy (music by Jule Styne),
However to answer your question of these are some of his best musicals that he wrote the score and lyrics to:

Into the Woods
Sweeny Todd
A Little Night Music
Follies
Company

Hypera has old the musicals Sondheim wrote, but before he was a lyricist/composer he also wrote tv shows like "Topper," and he also wrote a screenplay mystery with Tony Perkins.
