A douche is something use to clean your vagina. It's a small bag that you fill with a vinegar and water solution and squirt up there. Hence, doucheBAG. (And No, I've never used one. My vagina cleans itself just fine on it's own.)
Anyway, when you call someone a douche or a douchebag, you're really just saying that they are lame or an arrogant f#@%!

It the old days women used to clean themselves internally from an apparatus that was a bag and a tube. Now, the same idea, but it is a bottle with a nozzle.
