Why not just read the book and find out? It's more of a novella, and you could probably read it start to finish in a couple of hours.

Jamie DOES have leukemia, but Nicholas Sparks never mentioned that she dies.
Actually, the book is being narrated by a 57 year-old Landon Carter, who describes how he fell in love with the Baptist's daughter, Jamie, who was shy, plain, not stylish and religious. They come close because of a play (which was written by Jamie's father), and fall in love. Landon does everything he can to make Jamie be happy. Then one day Jamie tells him that she has leukamia. He gets devestated, and tries to stop her from dying. He proposes to her, and she agrees.That's how the book really ends.
Then the author leaves it upto the readers to decide what happened to Jamie: Did she die? or did she survive, for atleast a few years?
Nicholas Sparks did this because he loved the character of Jamie, and didn't have the strength to write that she died. So if you want to believe that she died soon after marriage, then she did. If you want to think that they started a family, and she recovered (or died 10-15 yrs later) then, yes, they did have a family and live happily. It's all upto you.
Jamie died in the movie. But the book is another thing.

I can't imagine that any teacher or professor worth his/her salt would actually assign this for reading, so I will go ahead and tell you what happened. It's a nice book to read, but by no means a piece of great or classic literature.
It is mostly set in 1958 - Landon Carter, as an adult, is basically locked in a memory during the book. Landon is a kid who I don't think is like a "bad boy" (as in the movie), though he is one of the most popular kids in school. Jamie is a girl who is very quiet, shy, and extremely religious - she is the daughter of a minister. Her mother is passed away. Landon is in the school play and asks Jamie to help him learn his lines, and they begin to get closer, ending up with the two of them falling in love.
Jamie finally tells Landon that she has Leukemia, and will not be getting better. Landon proposes to her, and they are married, and Jamie dies a few months later. Landon doesn't ever remarry after that (he is 57 when recalling the story). The lessons he learns from Jamie are faith, love, and the whole don't-judge-a-book-by-its-cover thing.
Trivia: The novel is based around and dedicated to Nicholas Sparks' sister, now deceased.
