It needs to affect you enough that you aren't able to work. It may be diffeerent in some states, but in North Carolina it is determined by the Social Security office. You have to fill out paperwork and get documentation of your disability from a doctor/psychaiatrist.

If you have a certifiable bi-polar disorder then have a psychiatrist give you the proper paperwork so you can submit it to the government. It takes time but you can get SSI if you can prove that you cannot work and earn a living. If you obtain SSI you will be reviewed every so often to make sure that you still cannot work.

It has to be bad enough so that you cannot work. The process is long and can take years.
