there has never been a UEFA Champions' League finalist from the same city in the entire history but there was two teams from the same country played each other in the UEFA Champions' League final (2000 - Real Madrid CF v Valencia CF & 2003 - AC Milan v Juventus).

It could have happened in 2005, if Internazionale and AC Milan had been kept apart in the semi finals. Alas, it wasn't to be. The other semi final was between Liverpool and Chelsea, and I'm pretty certain that this is the only occasion where each semi final tie was contested by clubs from the same country.
