The killer is a psychopath who actually wants to be a woman. He tries cross-dressing in front of a mirror but realizes that he is still 'in a mans skin'.
The reason why he kills women and starves them is that he wants to make himself a 'woman suit' from their skin after it becomes loose on their bodies and is easy to skin off.
In order to keep the skin supple and from tearing when he skins it like a deer, he wants the captive girl to put on lotion.

He has the girl in a well. He's trying to make an outfit using the skins of plus size women. He wants the girl to keep her skin nice and moist, so he lowers the lotion down in a basket, and thus the famous line.

haha..Eric Cartman plays a game like that in his basement a lot, he has a little dolly in a well and lowers lotion so she can apply it.
Basically there is a well where girls are kept and skinned alive so Bill can make a suite from their skin, and he wants it to be soft so he lowers them lotion to apply. "It puts the lotion on it's skin, or it gets the hose again" >.<

The whole thing about the lotion is that he wants her skin to be healthy so that when he tried to use it to sew together a suit her skin won't rip apart.... the lotion is simply being used to keep her skin from becoming brital and dry
