An example of a passive sentence would be:
"The apples were eaten yesterday."
An example of an active sentence would be:
"We ate the apples yesterday."
A passive sentence uses the noun recieving the action as the main subject, and an active sentence uses the noun performing the action as a main subject.
I'm pretty sure that's what it is, that's what I learned in school, anyway.

John(subject) reads(verb) the book(object) - Active sentence
The book(subject) is read(verb to be + past participle) by John - Passive sentence
The active voice is what we usually use when speaking. We are interested in someone performing an action to an object. In the case above "
In the passive we are interested in the action only, not the person who performs the action.
The passive is formed by using the object in the active sentence and making it the subject in the passive sentence, in this case "the book".
The verb in the active sentence "reads" is in the present simple so in the passive sentence the verb is "is read". The passive being formed by the verb to be + the past participle of the verb.
If it is important to know who is performing the action we can include BY at the end plus the person.
Therefore our passive sentence reads "The book is read by John"
