Suprisingly Disney (all parks combined) have the most accidents each year, more than Universal or Six Flags. Even though Six Flags isnt 100% Disney standards its just as safe, if not safer.

I'm not for sure about the total but I would say Paramounts Kings Island in Cincinnatti, Oh they had a coaster called The Bat that fell off the track and killed several. this happend in the 1980's
