I've never had the problem of having to find a sample clinical report because my professor always provided them, but I decided do a search and see what popped up.

I did a google search for "sample psychological reports" and the first 3 or so hits looked pretty good from what I know. They used the main components that are necessary to complete a psychological report and can give you an idea of the information you need:
