I research, write, create, design, print and coordinate the delivery of marketing brochures for my company. I know that I must put the doctors name primarily on the front cover of the brochures in order for the inofmation to be considered valid, but how do I give myself credit on these brochures? Where do I put my name, since I am doing all of the work. I would like to have some recognition by adding my name somewhere on the booklet. The back of the booklet has a picture of the dr. with his name, address, clinic name and contact information below it. Where do I put mine?

I don't believe I have ever seen the creator's name on a doctor's brochure before. I believe that's something you should talk to the doctor about. Or, you can always keep a portfolio of all the brochures you create for show when the time comes for a change of career.
