The old ones were attached using nails but I'm not sure that is a good idea.

Hard to say for sure without seeing it, but I would screw it in...

Use a hammer-drill and masonry bit to place a threaded concrete insert, that you set into place with a hammer blow. Then drill a hole in the cover to line up with it big enough for the mating screw. If you are concerned about vandalism, use a tamper resistant screw.

There are screws made just for "masonry" - Check with your local home depot or hardware store.

i would think the crawl space would have a wooden frame but maybe not, in that case i would use tap-cons
