When possession changes. If one team make a basket, the other team takes the ball out before they head to their side of the court. Also if there is a non-shooting foul, the ball is taken out by the team that was fowled, or if it gets knocked out of bounds it is taken out by the team that did not knock it out.

the ball is taken out after a made basket, after the ball goes out of bounds, after a non shooting foul, any penalty including offensive fouls, travels, double dribbles, or a kicked ball

pretty much after every turnover or penalty on the defensive team and to start each quarter

When a shot is made, the ball goes out, fouls that are non-shooting and the team is NOT in bonus, after a timeout, after a technical.

I don't really understand your question, but I get it like - when the player gets out with ball - so he is out as soon as he steps out of court with ball in his hands or touches ground out of court with the ball.
