There's a number of things that you can do to get your chances up for getting into philosophy program as an undergrad...

It's going to be VERY important to keep up the GPA, particularly your major will be vital. Most accepted applicants have a minimum GPA of 3.5 overall. Start studying for the GRE soon. While the status of the test is in question at some school, better score is, well, always better. While these are your base strategies, there are other things that you can do. Try to write a paper good enough to present at conferences or publish. This is the kind of thing you will be expected to do in grad school, and if you have done this as an undergrad, it bodes well for the future. Also, it is important to establish good relationships with your current professors. Not only will they be writing those all-important recommendation letters, but they can also serve as an excellent resource for advice on grad schools, publishing, etc.

As for "good" philosophy programs, I've seen this webpage referenced a few times, both with positive and negative evaluations:

While I would not take this information as the word of God on US programs in epistemology, it is a decent start. Perhaps most important is paying attention to the names you read in those academic journals. If you find there is a scholar whose work is consistently invigorating and enlightening, you should consider studying at the same program as this individual. Remember, grad school is an academic apprenticeship, and the person you study under matters greatly.

Best of luck with you inquiries!

Absolutely! On a word document, submit this age old question and answer it when you apply to grad school.

Question: What, if anything productive, can I do with a Philosophy degree upon graduation?

If you can answer that, you'll have the top grad schools beggin you to attend!
