My old landlord used some resource online to draft my rental lease, but I can't remember what she used. Does anyone know where I can find a virtually "fill in the blank" rental agreement?

Office supply store have blank forms for this. You can also get on your state's web site and read the laws regarding leasing - it will be an eye opener. The bottom line is - it has to be in writing! The condition of the place at move in. What appliances are present. Fees for damage. Terms for moving out. Terms for refunding deposit etc. The blank form is fine for a start, but needs to be tweaked for your state laws.

you can get a free lease agreement from Microsoft Office Online templates. It is from Findlaw!

Just like that. Specify that the lease with the roommate is on a month to month basis, and specify the rules that you have and all. even if you think that something is silly to put into this contract, put it in anyways. it can't hurt, plus it may save your butt in the end....
