Two come to mind at the moment.
Catwoman - She's always been my favorite Batman villain, and they totally screwed that one up.
Beowulf & Grendel - Hours and hours of absolutely nothing happening. It's based on, of course, the Norse legends of Beowulf, and this "monster" Grendel. The movie moved so slowly, and nothing ever happened. Ugh.
Update: Apparently I can't read, and thought this was a question about movies. Hmmm...embarrassed now...sorry. lol

Survivor.
I have yet to hear about someone that lost on Survivor actually passing away.
At least have a LITTLE bit of danger. Voting someone off - big deal. Push someone over a cliff or shoot them.
Make it a little bit exciting.

Mr. Extraordinary. It was a rip off of superman. Fortunetely it only lasted 2 episodes before it was canceled.
