Take a leaf or two to your local nursery. I can think of a few plants that might fit your description. How tall is it, what color green, any other colors, flowers? Shape of the plant? Is it woody, etc. where do you live? You can also try a gardening book from the library, too.

I think your plant is a lovely plant that trails from the pot called 'Chain of Hearts', botanical name is Ceropegia linearis. Google this and see if it's the same. They usually have Grey green and green variegated leaves, variegated varieties are also available.
