Firstly, decide if you have the expertise to manufacture and print the t-shirts and what capacity or range can you handle. If you are not able to do so yet, go and learn how to do so first.
Next, research on how to market your product. That means finding out who wants to buy what. What sort of material, colors, designs? Who would be your potential customers, where are they, where and when do they shop? Who are your competitors and threats that may affect your products?
Once your research is done, it should tell you what to sell, when to sell and who to sell to. Now decide how you want to sell it. Do you want to be mass manufacturer supplying retailers worldwide? Or a specialty maker printing single orders? Or open a stall/shop/boutique to sell your own limited-edition creations? Is there something special about your t-shirts that others don't have? Do you have an unique store or location that makes your t-shirts stand out? Or do you want to sell it by mail order or through the internet?
Once these (and many, many more questions are answered), then you have an inkling of how to start. But there are also many other details you need to think through before actually commencing a business, depending on how big you want to be. A good way to start small and see if your ideas/designs will sell is to open a shop at www.cafepress.com. They have both free level and pay level ($7.00 per month ) shops.
Starting there allows you to design and sell your shirts without investing in tons of equipment. After you place designs in your shop Cafepress handles everything after an order is placed. They print/ship/collect the $ and then pay you a commission on the sale. If your stuff sells well then you know that it might be worthwhile to get some equipment and do it yourself.

Decide how to & with what form of decoration mediums you wish to use.At first always limit quanity of sizes & colors of T-shirts.Go to a hobby store to buy your T-shirts & supplies or fabric store & buy fabric & patterns.Once you have them made sell them at (any or all) swap & shops,flea markets,craft sales &/or yard sales. Good luck!!! have FUN This is how I did it.
