I always tell myself to wake up during a dream if I don't like the way it is going, so I would have to say that most the time I realize when I am dreaming. I don't know how to help you other than try to test the dream while you are asleep.

You can, it is called lucid dreaming, when you have a lucid dream you can also chose what you want to do in that dream (i.e. flying). You can train it: In an everyday situation think of something impossible to happen (for instance, you are at lunch with a colleague and you imagine him starting to bark, but I'm sure you can think of many other things). Now if you do this quite often, this situation eventually will come up in a dream, if indeed this happens you know you are dreaming.

I'm not sure what you are talking about... I almost always know that it's a dream while I'm dreaming it.
