Depending where you live ..go to the closest taxation centre in your area and aske them for a copy of your 2005 taxe forms and they will give them to you free of charge all you will need is two piece of Identification with picture..plane and simple bud.If you don't live close to a taxation center then you will have to find either an address to one or a phone number and ask they send it to you via mail..
