Why don't they start at 10pm all across the US?

That is because 10 PM EST is 9 PM CST. It is much easier for a large cable company to start the program and play it once for both time zones than to have to mess with playing it twice, once at 10 PM EST and once at 10 PM CST.

Sorry, Sammy, but your chain has been yanked. When it's 10 PM in the Eastern time zone, it's 9 in the Central, 8 in Mountain, 7 in Pacific. Because of the difference between Eastern and Pacific, the shows are shown simultaneously in Eastern and Central, and later in Pacific.

That messed me up when I visited my buddy in St Louis. He said It's because the Midwest has most of the farms and farmers need to get up early and go to bed early (to slop the hogs I guess), so their shows need to come on earlier.

He could of been yankin' my chain, though.
