One for sure if through the air cleaner and another possibility is a blown head gasket

I know one way is when you change your plugs there was dirt around the plugs and the dirt fell into the cylinder holes where the plugs go, and sucked down into your crankcase, dirt can also do damage to cylinder walls and piston rings.

If you have a dirty air filter, it will let dirt enter the crankcase. Dirt can enter through the pcv valve also.

Any time you open the fill cap or pull the dipstick. Also any loose or missing components connected to the engine.
