Well, according to the US census bureau the three most impoverished counties in American are all in South Dakota.
Ziebach...Shannon...and Todd Counties.
Keep in mind poverty is a family of four that makes $20,000 a year or less (which may be easier to live on in south dakota than it would be in the bronx or the southside of Chicago or Cleveland)
Since the census breaks things down by states,counties, cities (major ones only) and not neighborhoods/burroughs its hard to acurately quantify them inside of a city's statistics.

they have an interactive map (click on map to get finer and finer resolution) where you can examine a variety of variables, including median income with the resolution of census tracks. But the smallest category is <$20,000. So you can't find the poorest but you can find where the poor are concentrated in cities. The raw data is available in a huge data base, but you have to know how to use it. On a large scale the poorest areas are in North and South Dakota, I think they are Indian reservations. However most large cities have neighborhoods that also have median incomes <$20000. An oddity of the way data is collected is that census tracks that are mostly college campuses show up as poverty areas because students have little or no income.

I looked it up on wikipedia and it is Cleveland, Ohio according to the United States Census Bureau's 2006 population estimates.

I am sure there are far to many to nail it down to just one. If you only knew how many families are living below the poverty level, you would be amazed, even in neighborhoods you would least expect it. It is worse then most are aware of.
