It is an enthusiastic acceptance to an invitation. Example: You are invited to a party to announce a couple's engagement to be married-- and they are very dear to you and you are happy about it. So, you'll be there with bells on in order to help celebrate.

It means happy and delighted to attend.

Goes back to the days before automobile, when it was the custom to use your fanciest harness (with bells) for your horse, when it drew the carriage for special occasions.
