I haven't known him since I was a baby and am having problems getting the information from other people

You don't give your country, so I'll assume you want a US passport and you are completing form DS-11.
You could complete the boxes you know the information for and write UNKOWN in the other boxes.
There must be lots of people who don't know full details about their parents.

If you want to get a passport, you probably have to provide your own birth certificate. Isn't your dad's name and birthdate mentiond there?

You can fill out what you can, but you will have to submit an "original" birth certificate when you apply, and the birth certificate will go along with the application, and when you get your passport, you will also be returned your birth certificate.

no, you only need your personal info:birthdate and all you ID's, go online and find out more about it. I don't know much about my father either but I have all my IDs and never had to put down anthing at all about my father. Sometimes forms will ask your mother's maiden name, but nothing about their birthdays or anything.
