You should also be asking single, divorced and widowed people too to get an accurate picture.
(1) What relationship were your parents in when you were born?
(2) What relationship were they in when you turned 16/18?
(3) What relationship are you in now?
(4) What religion are you, if any?
(5) How many weddings did you attend in the last year?
(6) How many divorces /separations did you hear of (family/friends only) in the last year?
(7) (Single people only) Do you plan to get married?
(8) What do you believe is the main cause of divorce?

Im a little over your top age requirement, sorry, but think I can help you out as I have dealy with and deal with alot of troubled married couples. 1. What does your marriage really mean to you?, 2. Why did you really want to get married?, 3. Are you willing to do whatever it takes to make your marriage suvive the good and the bad times?, 4. What makes you so sure your spouse is the one you wanted to spend a lifetime with?, 5. Do you think children will make your marriage stronger or weaker and why? 6. If an "affair Situation" presented itself, would you think of entering it? 7. What do you think your best assets to this marriage are?, 8. Who would be your best contact to go to in times of need or trouble? How do you think you can make your marriage better?, 9. What exactly is it you want out of your marriage now and in 5, 10 20 years? 10. Who would be your role model? I can think of hundreds more but this should get you started. Good luck

how often did you talk to your wife / husband about .......... when you first got married ? (with options)
then the same for now
that sort of thing. you could ask about time spent with partner, level of satisfaction with the marriage etc
