Pearls come from oysters. A grain of sand (or some such thing) gets inside the oyster and the oyster doesn't like it but it can't spit it out so it coats the sand grain with layers and layers of stuff to smooth it out. Now you have a pearl. Most natural pearls are not perfectly round because most sand grains are not round. Cultured pearls are made by inserting little perfectly round pieces of plastic into an oyster which the oyster then coats to make the pearl.

Pearls can come from any bivalve (oysters, clams, scallops, etc.). Typically, people think of pearls coming from oysters. Since the bodies of these creatures are soft, sand and other hard debris are problematic as they can scratch and damage their tissue. Therefore, they have evolved the ability to "wall off" the offending grit with a substance called "nacre" which can also be found on the inside of the shell (AKA "mother of pearl"), that shiny pretty surface you see on the shell interior. The animal secretes nacre to help his shell grow bigger as needed, its usual purpose. This nacre will build up on the tiny bit of grit and over time the nacre-covered grit piece become bigger and bigger, becoming what we recognize as a pearl.

Now, what if you could never take your shoe off?

Your species might, over several thousands of years, develop the ability to secrete some goo to smooth that stone off, so it doesn't hurt when you walk.

A pearl is a grain of sand surrounded by the secretions that oysters (and some similar species) have developed to make their shells habitable.

We humans have a similar tactic... when we make calluses to deal with where we're being injured a lot. Oysters make the thing irritating them less scratchy, we make the skin touching an irritant less sensitive.
