No. It's just a lap dance. More erotic, for most, than just watching someone on stage, but as it isn't an actual sexual act there's nothing wrong with it at all. If there is any sexual contact going on then it's obviously not just a lap dance anymore. All the dancers that I know of who dance even put a little towel down on the customer's lap first.
And no, there should be no touching on the man's part whatsoever. That just gives opportunity for misinterpretation and is a safe-guard for both the dancer and the customer. Why give someone a chance to complain about it, rightly or wrongly, and ruin it for everyone?

No, why? I've never been to one and I don't see why they should. They have enough bouncers there to handle bad situtations. And obviously the woman doing the lap dance agreed to and the man receiving asked for it. Don't see anything wrong with them.
