Mental illness, like many other things in life, is not a black and white issue... it is more grey. That is, mental illness exists on a continuum and sometimes people are at the end of one side of the spectrum, and at other times in their life, they fall into the other side. Granted, if one were to use the DSM-IV-TR (APA, 2000) to try and identify mental illness, one would surely find a diagnosis that described almost everyone they know. But DSM-IV-TR simply describes clusters of behaviors.
Thus far, science has not produced any true empirical means for testing for mental illness. There is no definitive blood test, brain scan or other medical procedure that can "detect" such illness. In my opinion as a therapist in the field, I can tell you that behavior must be looked at in context with the environment and culture of the individual. And yes, there are some people who might be considered "mentally" ill; but i think that the majority of people lie somewhere along that continuum, and the real answers lie in learning to identify where you are on that journey for yourself.

All people are mentally ill at some point (and if you see the manual referenced above, I believe it is more like all people have some form and some degree of some mental illness all the time).

As for the change over time, if you've ever had a really major disaster (physically or mentally) happen in your life -- were you aware of your actions? or even where you were most the time?
Or for another example: post-pardum depression or other hormonal imbalances....

If you saw the book DSM-4 the diagnostic and statistical manuel on psychiatric disorders you would not be asking this question.
