This is all fungus of one sort or another. Do a water change of about 50%. Make sure that you dechlorinate/detoxify the water first. Treat with Maracyn or Maracyn-2 according to directions. Take the carbon or other filtering media out of your filter casing as this will remove the medication from the water, but keep your filter running. Treatment can take a very long time, so a lot of patience and diligence are required. It won't get better in a few days. Also, medications disrupt the nitrogen cycle, so, when you stop treatment, you will need to recycle the water. This can best be achieved by using Cycle or Bio-Spira. Diseases are almost always caused or made worse by poor water quality. You will constantly have diseases until you address and correct your water quality problems. Learning how to do proper water changes and tank cleaning will correct these problems. Learn about the nitrogen cycle. Learn how to test your water so you know when you have achieved healthy water. it sounds to me like you have a fungus problem. You need to go to the store and get an anit-fungus medication. You will also need to do about 20% water changes daily. This might or might not work. The eyes are cataracts, which is a fungus problem, the mouth is called cotton mouth, another fungus problem and the fin rot is due to the low immune system, which is due to all the rest of the problems going on with the fish. your fish has ick you can get ick medicine and put it in your tank after you clean the tank.it helps the fish to start getting better.
