Is there a question here somewhere? You've typed a partial pleading header. The name of the county appropriate for the case at issue should be put there. That would normally be the county in which the event that is the subject of the case took place. Please flesh out your question. If you just need to know what a pleading is supposed to look like, go to your local law library and check out the local rules. It'll be a fat book with all the information you need to file a lawsuit. Or, you could hire a lawyer. That would probably be the smart thing to do.
