i would say "boss i have a neccesary appontment at dat da da time and would like your permission to leave early today. i realize this may put me behind on my work but i am willing to make it up another time when you need me to stay longer."

You have a doctor's appt that you just made and you're not feeling well, and if you don't go today you have to wait until tomorrow, and it's really important. Or... you have to pick your kid up from school early. Or... you have to pick your sister's kid up early...
