Only Brian (the dog) can actually understand the words that he's saying, but the family understands his mood. So if he says "I want you to burn in hell" than the family would just think he was angry and needed to settle down.

Only Brian, the family dog, can understand the words that are coming out of Stewies mouth. This is so silly because everyone in the world understands Brian, he even got a job as a screenwriter at some point.

It's suppose to be that only Brian the dog can understand. But I think it's one of those things like where are the Simpsons really from?

Brian (the dog) understands him word for word. The rest of the family seems to get the gist of what hes saying but not his exact intent.

most of the time only Brian can understand him, but I have seen episodes where it seems the other kids and Peter can understand him, though I've never seen it with Lois.

I think that when he is doing all of his bitching, that they only here him crying. But, that dog can understand everything that he says.
