My Fiance and I both smoke so i have this problem also. I spray my seats with Carpet/Upholstery cleaner and use a damp sponge on them. I will also spray my Car occasionally with a vanilla Musk scent to make it smell good. Spraying Fabreeze will also eliminate odors.

Resolve foam and a stiff brush works great on cloth seats. It got a mocha stain out of my seats. Also Febreeze makes a spray specifically for cars. It works really well. I spray it on the seats, carpet and the liner.

well if the seats are removable take them out and use a steam carpet cleaner if not you just have to us it inside the car but keep the windows down and it will come clean, just don't use the car for a few hours unless you want a wet butt.

Rent a carpet cleaner and clean them. Do it on a really warm day when you can leave the windows down to dry or leave the car running with the heat on full blast for about an hour.
