I am good for about one to two days then I get slack and give in. I start to create the new me one day and wake up the next day only to go back to the drawing board I am constantly cursing myself and feel depressed because I always let myself down by procrastinating. How do I greatly improve my discipline levels?

This method may not be popular but it works. Answer these questions:
Who was the wisest man to ever live?
Who was the richest man to ever live?
Who was the best businessman to ever live?
Who was the best author/songwriter to ever live?
Who was the most successful politician to ever live?
Hint the answer is the same person for each question.
The answer is Solomon, King of Israel. and he wrote a book called Proverbs, it is in the Bible and it contains 31 chapters and each month has 30 or 31 chapters so if you read a chapter/day that corresponds to the calender date, you will read Proverbs about 12 times per year. This book is written as a wise loving Father giving advice to a well loved child and becomes a good guide to success and over a period of time you will recognize mistakes you are making and be able to correct them. You will also see mistakes others are making and see the unhappy consequences of their choices so you will be able to trust what you have learned in Proverbs. There will never be a time in your life that you won't be able to profit from good advice and doing this simple exercise of reading Proverbs daily will improve your life forever and enrich those around you.
I did not invent this idea but it has been practiced by many great men in western history so this system has a record of success

Try organizing your plans by making a list of steps to do. I can't guarantee it would work for you because it didn't for me, but just try in case it does.

What I do is formulate the plan in my head and keep going over the steps like a mantra, telling myself in simple steps what I had to do. Usually I think about the steps just before dropping off to sleep at night, so it gets registered in my mind. For instance, if I tell my mind if I have to wake up at 5 and study this or that (I'm a student) from 5 to 7 and this or that from 7 to 9, and complete the thing before 9, I'm usually successful in the job. It works like magic.

The trick is to take things little by little. My biology teacher always says that rather than aiming to get a full mark on your test paper, its better to aim to get a full mark for each question of 1 mark, or 3 mark or 5 marks, and things will be much easier. Make sense? So instead of formulating your plans for the entire week, construct them for one day by one.

Surround yourself with reminders of your goals. If there is a place you want to live, put up a picture to remind yourself of it. If it's a career you want, put up things that have to do with that particular job market. It might sound silly, but it does help to keep you motivated and keep your goal in sight. My wife did this when we were trying to move to the beach. She put up pictures of beach scenes everywhere, in the kitchen, on the bathroom mirror, in the bedroom. Trust me, every day we got up and felt like giving up the struggle, we would look around and remember what we were working so hard for. Yes, we got to the beach and lived there for a year. It was awesome and we were both so proud of our accomplishment. You can do it. Try it. Good luck and remember to never lose sight of your dream. Anything worth having requires hard work to get.

Allow yourself some downtime to regroup..Constant demands on yourself can be meaningless if it defeats your purpose...good luck!
