There are two instances when volcanoes significantly affected humans.
1.) Mt. Vesuvius in Italy, the worst ever eruption to take place in history, erupted on 24 aug 79 AD burying 2 big cities Pompeii and Herculaneum under its ash.
2.) Mt. Karakatoa in Indonesia, produced the loudest eruption in history, on Aug 26,1883 and shot 25 cubic km of ash and rock into the atmosphere.The explosion was hard as far as Perth,Australia and Mauritius Islands. It took more than a year for the dust to settle down and on day of eruption. It destroyed around 160 villages and towns and killed more the 36000 people. the moon even appeared blue....giving rise to the term ... once in a blue moon

There has been heaps of them - all varying in degrees. Mt St Helens is a recent example - but luckily the death rate was minimal - though the forest didn't get away.
Going back in time there have been pre-population eruptions that must have been just awesome! But here are some that may be of interest, if only because of the rate of carnage and death that resulted.
Santorini - Greece. This eruption was so great it took nearly 2/3rds of the island under as well as totally covering Crete in over 40 feet of volcanic ash.
Mt Vesuvius - Italy - 79AD. Killed most of the inhabitants.
Kelut - Indonesia - 1586. Mortality rate: 10,000.
Lakagigar - Iceland - 1783. Mortality rate over 9,000.
Unzen - Japan - 1792. Mortality rate over 15,000.
Tambora - Indonesia - 1815. Mortality rate over 92,000.
Krakatau - Indonesia - 1883. Mortality rate over 36,000.
Mont Pelee - Martinique - 1902. Mortality rate 30,000.
Nevada del Ruiz - Colombia - 1985. Mortality rate 25,000.
Most of the fatalaties were resultant from either: tsunamis, mudflows, pyroclastic flows, suffocation from ash and dust, starvation.
