Most likely a survey was done on breastfeeding. Perhaps each year less, and less, African American mothers are breastfeeding. Compared to other races. This could be that more African American Women are now working out of the home, and have less time to breastfeed their babies. So their is a decline.

I think it means compared to other mothers of different ethnicities, it seems as though black mothers are not breastfeeding as much as other mothers of a different race. So in fall behind, it means not as much.

it means that black mothers are not keeping up with their white counterpart in the effort to breast feed.
