Yo Darlin
You need to get in touch with your local IRS office and get a form from them for this !! You will be asking them for a form that has a Form Number-----1040EZ-T
They should have these readily available -- the form is self-explainatory (odd as THAT might seem) and you should have it done in absolutely no time !!!

If you are filing a tax return, you can claim the refund on your 1040 or 1040A or 1040EZ - there's a line for it in the payments section. The easiest thing is to take the standard refund, which is $30 for a one-person household, $40 for two, $50 for Three, or $60 for a four or more person household. If you prefer, you can dig out your old phone bills and itemize the taxes from March 03 to July 06, and take the actual amount - then you'd need form 8913 to list the detail.

If you are NOT filing a tax return but are entitled to the telephone excise tax credit, file a form 1040EZ-T. Do NOT file this form if you are also filing a federal income tax return.

If you do your taxes on line and take the standard deduction of $60.00, you won't need all of your bills for the past years!!
